Uses and Disclosures That Require Your Written Authorization
Other uses and disclosures of your PHI not described in this Notice will be made only with your written authorization. This includes most uses and disclosures for marketing purposes and any sale of your PHI. If you give us an authorization, you may revoke it in writing at any time. Revoking an authorization stops any further use or disclosure under it, but does not affect disclosures we already made while it was in effect.
Your Rights
You have the right to:
- Request restrictions on certain uses and disclosures of your PHI. We are not required to agree to most requested restrictions. However, if you pay for a service or item in full, out of pocket, you may ask us not to disclose PHI about that service to your health plan, and we must agree unless the disclosure is otherwise required by law.

Under the law, health care providers need to give patients who don't have insurance, or who are not using insurance, an estimate of expected charges for medical items and services before those items or services are provided.
- You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services, provided upon scheduling or upon request.
